Thrivent Giving Plus Application
Click on the link above to view and/or print the Giving Plus application that needs to be filled out when requesting matching funds from Thrivent for a monetary donation made to Immanuel Lutheran School. If you wish, you can use your keyboard to fill out the form while it appears on your screen; just be sure to print a copy of the form BEFORE you close the window that the form is in. The filled-out form should then be submitted to Immanuel's office with your donation.
